장골이식과 평판에 의한 법랑아세포종의 치험례

Abstract
1. 본 증례는 17세 여자의 과두를 포함하는 우측 하악 구치부 및 하악지부위에 발생한 법
랑아세포종으로 하악골 적출술후 인공과두가 달린 A/O 금속평판(A/O metal Plate)와 장골
이식을 통해 즉시 재건하였다.
2. 술후 3 주경에 감염을 경험하였으나 적절한 치료로 이식물의 제거를 막을 수 있었다.
골이식을 하는 수술에 있어서 무엇보다도 감염에 대한 주의와 예방이 필요하며 감염이 된
경우에는 빠른 절개와 배농으로 이식물의 희생을 막을 수 있었다고 사료된다.
3. 발생빈도가 드문 17세에 발생하여 성장이 다 끝나지 않았을 가능성 많았으나 술 후 4
년이 지난 현재 하악골의 편위나 이환측의 악관절이상이 발생되지 않았으며 교합상태도 양
호하였다.
4. 파노라마(Orthopantomogram)를 통해 3∼6개월 단위로 관찰한 결과 술후 2∼6개월에는
골의 흡수, 형성이 왕성하였고 6개월이 지난 이후 방사선 불투과성의 증대 및 골형상의 분
화가 두드러졌으며 1년 이후에는 큰 변화를 보이지 않았다.

This is to report a case of immediate reconstruction after hemimandibulectomy by
using of bicorticocancellous block bone harvested from the iliac crest in the case of an
ameloblastomaon the mandible. Because the lesion involved condylar area, it was
reconstructed with titanium artificial condyle attached to A/O metal plate. Three weeks
after the operation, infection developed with suppuration and was well treated with
adequate antibiotic therapy and drainage. The patient has been followed up over a
four-year period and taken an orthopantomogram every three or six month for the
examination of mandibular movement, the potentiality of recurrence and the remodeling
of the grafted bone.
At present, the patient is satisfied with her appearence and has a normal occlusion
with proper masticatory function, and there is no sign of recurrence.
